Affirmative Form: Subject + have/has + past participle Example:

Negative Form: Subject + have not/has not + past participle Example:

We have not seen each other for a long time.

He has not traveled abroad.

Interrogative Form: Have/Has + subject + past participle? Example:

Have you ever been to New York?

Has he read the book?
